Third Party referrals
We accept referrals for DVA, NDIS, Work Cover, Insurance claims and Medicare Care Plans.
It is you and your GP’s responsibility to make sure your referral has been sent to us and are valid.
If we have not received your referral, you will be asked to pay up front and the responsibility to claim back will be yours.
Medicare Referrals
Our senior physiotherapists offer Medicare rebates.
Please call us if you require a Bulk Billed appointment as not all of our physiotherapists offer this service.
It is you and your GP’s responsibility to make sure a Care Plan has been sent to us, entitling you to receive treatment under this government arrangement.
If we have not received the Care Plan, you will be asked to pay up front and the responsibility to claim for Medicare will be yours.
You are eligible to receive 5 (Five) Medicare Funded treatments in a Calendar Year for allied health.
You must inform us if you have received treatment or been referred for anther allied health service under a Medicare plan, as it will affect how many appointments Medicare will cover, and cause rejection of your claim.
If for any reason your claim is rejected by Medicare, payment for the account becomes your responsibility.

Authority to Obtain Information
By attending your appointment, you consent for the physiotherapist to collect all necessary personal and medical information that may be relevant to providing a quality service. I am aware that any information withheld could affect my ability to participate and may result in injury.
I consent to my treatment notes being shared between all Sports Med NQ and Core Movement NQ Clinics for continuity of my care.
I agree to accept full financial responsibility for all of my treatment/services rendered.
I consent to my physiotherapist and/or exercise physiologist contacting my GP to share information relevant to my injury and treatment.
Communicable Diseases
If you have contracted a communicable disease, please ensure you have clearance in writing from your doctor before attending your next class or appointment. This is for the safety and well-being of all our clients, staff and customers.
Personal Agreement
Core Movement NQ does not assume responsibility for any lost or stolen personal property. If you do become the victim of a theft, immediately report it to a Core Movement NQ staff member.
Core Movement NQ does not offer childcare services. Mother’s who attend the Pre and Post Natal class are welcome to bring their child along to class.
Class Bookings, Attendance & Payment
All new clients must attend an initial assessment with one of our physiotherapists, or have a referral letter.
Bookings are made via the MindBody app. If you are having difficulties, bookings can be made via phone or in the clinic.
To book into a class you must have pre-purchased session or pass.
Passes are valid for 12 months from the date of purchase and are non-refundable and non-transferable between individuals. They cannot be extended except in cases of injury, for which a medical certificate is required.
Instructors reserve the right to restrict clients to a particular class type for safety reasons. Instructors reserve the right to turn clients away if they do not have the correct medical clearance.
Classes may be physically strenuous and there is risk of personal injury, property loss or death.
Clients must state any medical conditions or issues to the instructor prior to the class.
By booking into a class you automatically agree to the above Terms & Conditions.
